Tonmya: A New Fibromyalgia Drug Promising Sleep… But Will the UK Ever See It?

Fifteen years. That’s how long it’s been since anyone bothered to give fibromyalgia patients a new drug. Fifteen. Years. And now the FDA in the US has finally approved something new: Tonmya, a little pill you pop under your tongue at bedtime, designed to help with pain and non-restorative sleep.

On paper? It sounds like the holy grail. In reality? Let’s not get carried away just yet.

The Fibro Sleep Curse

If you live with fibro, you’ll know exactly what “non-restorative sleep” feels like. Spoiler: it doesn’t feel like sleep.

I can sleep all night, plus another 3–4 hours in the day, and still feel like I’ve run a marathon backwards. I’m conscious through my sleep, half-awake, half-asleep, waking up constantly. Sometimes I even get the joy of lucid dreams — fun for about five seconds, then it’s just exhausting.

I would genuinely kill for a proper night’s sleep. Just one. Imagine opening your eyes and actually feeling refreshed. Imagine not needing naps that last longer than most people’s work meetings. So when I read that this new drug supposedly targets exactly that — the broken sleep that defines fibro — you bet my ears pricked up.

What We Know About Tonmya

Here’s the science bit (don’t worry, I’ll keep it brief).

  • What is it? Tonmya is a sublingual (under-the-tongue) version of cyclobenzaprine, a muscle relaxant linked to tricyclic antidepressants, reformulated to be taken at bedtime for faster absorption and fewer side effects.Source

In normal speak: Tonmya is a low-dose, sublingual version of cyclobenzaprine (a muscle relaxant that’s been around forever), reformulated to be taken under the tongue at bedtime.

The idea is that this delivery system reduces side effects, gets into your system faster, and crucially, helps with non-restorative sleep — that weird not-quite-sleep state fibro people know all too well.

In trials, patients reported improvements in pain, sleep quality, and overall daily functioning. It’s not a cure. It’s not even pretending to be one. But it’s marketed as a targeted treatment for the “broken sleep + pain” combo that defines fibro. And honestly? That’s more than we’ve had in 15 years.

Now, before anyone gets carried away with miracle talk — let’s remember the dark side. I once wrote about my run-in with good old mirtazapine. It was meant to help, and instead it gifted me medication-induced Long QT Syndrome (cheers for almost killing me, by the way), which was only caught by pure chance. So yes, I’m wary of new drugs being hailed as saviours. Wary but curious, which is basically the fibro patient default setting.

Over here in the UK, Tonmya hasn’t even been submitted to our regulators (the MHRA), let alone made it to NICE to decide whether the NHS should fund it. Translation? Don’t hold your breath. Even if it gets fast-tracked tomorrow, we’re still looking at years, not months, before it ends up in an NHS prescription pad.

So while American fibro warriors are cautiously celebrating, the rest of us are stuck with the same old options: amitriptyline, duloxetine, pregabalin… and a heavy dose of “have you tried yoga?” from doctors who definitely don’t have fibro.

Cautiously Hopeful (But Mostly Cynical)

Part of me wants to be hopeful. A drug that acknowledges sleep as the core issue in fibro? Yes, please. If it actually works, it could be life-changing.

But another part of me has been burned too many times. The treatments we do get are scattergun at best. Exercise, CBT, antidepressants… it all feels like throwing spaghetti at a wall and hoping it sticks. And then there’s the price — because you know if it ever makes it here, there’ll be an almighty debate about whether our pain is “cost effective” enough for the NHS to bother.

The Bottom Line

So yes, I’m watching Tonmya with interest. I’m hopeful, but with the kind of weary sarcasm you develop after years of managing fibro on scraps of medical interest. Because honestly? I don’t need miracles. I don’t need a cure.

I’d just like one good night’s sleep.
